“Welcome, slaves of the lamp!” – This would probably be the most accurate salutation to people participating in replacement maintenance therapy program (RMT). For those who are offended, we would like to recall the plot of the soviet movie called “Aladdin magic lamp”, where the almighty djinn used to call himself the slave of the lamp, as he had to live in a cooper lamp and fulfill its owner’s wishes.
Most of the post-soviet territory population get wrong considering RMT a gratuitous lifetime drugs grant, free of any sense and bringing along the “narcotization” of the population. It is no surprise, as only those who have many years of experience in taking opiates are able to think straightly on the RMT topic.
Abrasions on the veins, they never will be healed
A soul covered with wounds, tossing in the grip
Darkness is behind, only fear ahead
How shaky is this phrase – you still need to live
This is and extraction from the posthumous book of poems written by Aleksey Perminov, a poet and “Slaves of the lamp” bandmate, he did at the age of 24 from heroin overdose. Opiates make a human feel almighty for a short time period, but then they enslave his soul completely and become the absolute owner of his life. Pain, fear and despair are the permanent companions of the addicted person, he is equal to any crime in order to “get stoned” and avoid the pending withdrawals. Death from overdose at a young age and the concurrent conditions are the sorrowful destinies for most of the opiates addicts.
RMT, on one hand, does not allow the withdrawal syndrome to develop fully, on the other hand, it prevents psychological addiction precipitations (cravings), constant relapses (slips), and resuming to drugs usage. Upon that, methadone (the main substance used in RMT) blocks the opiate receptors in such a coarse manner, that the patients, being inside the program, cannot achieve the euphoria feeling even taking an additional dose of heroin, codeine or street meth, which, consequently, makes the reception of street drugs senseless.
Considering the above, RMT is obviously a progressive treatment method, which stabilizes the patient’s health state, reduces the chance of their criminalization, and contributes to resocialization (return to the society). Basically, this is the first step in the personal drugs battle, and a very important one.
However, we should not forget that in order to achieve real blockage of opiate receptors, the dose of methadone required suppresses the body in general, induces drowse, dullness, apathy. Until now, the scholars did not arrive to a consensus on the hepato-neurotoxicity of methadone, as well on his ability to accumulate in the body and lead to mental disturbances on long administration. Moreover, methadone is still a narcotic substance. When discontinuing the drug administration, the person gets a withdrawal syndrome, even more striking and long that those from heroin or other opiate drugs withdrawal.
In “Aladdin magic lamp”, at the end of the movie the djinn, in order to switch from a bad master to a good one, leaves the cooper lamp and gets inside a clay jar, but all the same, he stays a slave. The addict, participating in an RMT program, is free of many nightmares associated with opiate usage, but still, he is an addict.
